Claims
A claim is a request for coverage or reimbursement for a loss incurred. The City of Eugene provides coverage for:
- Liability Claims, related to injury or damage to third parties occurring from City operations
- Workers' Compensation Claims for employees who have been injured on the job
- Unemployment Claims, for eligible individuals previously employed by City of Eugene
Liability Claims
The City of Eugene administers a Liability program to insure damages that may occur as a result of City operations. The City is self-insured and purchases supplemental insurance for excess liability.

- Administer, manage, investigate, and adjust claims arising from municipal operations
Filing a Claim
- To request a claim form, contact the Claims Team by phone or e-mail.

- Claims delivered in person or by mail will be recorded on the date received by the Employee Resource Center. Claims faxed or e-mailed and received before the close of the business day will be recorded as received on that date. All claims delivered after business hours (5 p.m.) will be recorded as received on the next business day.
Workers' Compensation
As a Total Worker Health® employer, it is our intention to create safe work conditions for all employees through regular safety assessment and preventative action. When an injury does occur on the job, our Workers' Compensation program assists injured employees to recover their health and productive employment as soon as practical.
The ERC Claims team provides claims management, information, and assistance to employees in all departments. Claims are administered through Cannon Cochran Management Services, Inc. (CCMSI).
Major Incident & Injury Reporting
- For any injury beyond first aid, employees should notify supervisors as soon as safely possible.
- During normal working hours, call ERC Claims Team at 541-682-5682 or 541-682-5061.
- After hours, call the Police Department’s non-emergency at 541-682-5111 and ask for ERC's emergency contact information.
- All such injuries should be submitted on a complete 801 form (PDF) and returned to Risk Services immediately.
The City's Workers' Compensation Program and OR-OSHA have stringent reporting deadlines. The ERC Claims Staff are required to submit complete information within 24 hours for hospitalization or 8 hours for on-the-job catastrophes or fatalities.

Minor First Aid Care & Reporting
- Employees should notify supervisor as soon as safely possible.
- Should first aid be required, contact Cascade Mobile Health to provide onsite injury response. This service is available 24/7, with professionally trained medics for non-life threatening injuries at the workplace. Call 541-228-3111.
- Complete entry in your department or division's minor injury log. If ongoing medical treatment is needed and you would like to file a claim, complete form 801 and return to the Claims team.

Unemployment
The City of Eugene's funds Unemployment through the State of Oregon to replace part of the lost income experienced following separation of employment with the city. Unemployment Claims are made through the State of Oregon as the benefit provider. The State's Unemployment Insurance Claimant Handbook provides detailed description of criteria for eligibility and instructions to file a claim.
Eligibility
There are two ways to qualify:
- You've been paid at least $1,000 in subject wages in your base year AND your total base year wages are at least one and a half times the wages paid in the highest calendar quarter of the base year. OR
- You've worked at least 500 hours and were paid some subject wages in employment during the base year.
Filing a Claim
Your claim can be filed online with the State of Oregon.
